How to request deletion of your Patient Diagnostics app account and associated data.

Last updated: September 22, 2026

Choose how to request deletion

  1. In the Patient Diagnostics app

Use the account-deletion option and follow the instructions to submit your request. Removing the app from your phone by itself does not delete your account or information stored by the service.

  2. Through your physician or practice

Ask the physician or practice managing your monitoring program to request deletion of your Patient Diagnostics app account and associated data.

  3. Email Patient Diagnostics

Send a request to [email protected] with the subject “Patient Diagnostics account deletion request.” If possible, write from the email address associated with your account. You can also contact us if you cannot access that email address. No app login or reinstallation is required to start the request.

    Request account deletion

If an email app does not open, copy [email protected] into your email service and send the request there.

What happens next

We verify your identity and authority to make the request, coordinate with your healthcare practice when needed, and explain the outcome or any retention requirements. We will respond within applicable legal timeframes. Please do not send passwords, login codes, Social Security numbers, or medical records by ordinary email.

Deletion covers the app account and associated personal information within our control, including profile information, monitoring data, and app communications, except records that must be kept for a legitimate legal, medical-record, security, or similar purpose. Information in backups is handled through the applicable backup lifecycle. Any required retention depends on the record and applicable obligation; we will explain relevant exceptions in our response.

Your physician’s medical record may need to remain available under healthcare recordkeeping rules. Deleting your app account does not necessarily remove records already held by your practice. Ask your care team about continuing care or monitoring before discontinuing app use.

Questions or a narrower request?

You can also ask about access, corrections, or deletion of particular information. Read the Patient Diagnostics privacy policy for more about how information is handled.

Website terms

This website provides general product and business consulting information. Product images show illustrative Patient Diagnostics branding. Final device configuration is confirmed with your order. Product availability, specifications, connectivity, prices, and ordering requirements must be confirmed before a purchase.

Submitting an inquiry is not an order, a prescription, enrollment in a monitoring program, or the creation of a patient–physician relationship. Website information is not medical advice. Your treating clinician can advise on device suitability and use.

This website does not promise particular clinical outcomes, insurance coverage, reimbursement, or compatibility with a particular system. Clinical monitoring and emergency response are not provided through this inquiry form.

Consulting and virtual support services, responsibilities, fees, and timelines are confirmed in a separate engagement agreement before work begins. A consultation request is not a consulting engagement, and financial or practice outcomes are not guaranteed. Company-specific purchase terms, returns, warranties, and shipping information will be provided with actual product offers. Contact [email protected] with product or website questions.
